By checking the "I Agree" box below, I certify that I am an authorized user and/or cardholder of the credit card provided on this form and that I authorize Divine Designs Salon Inc DBA Divine Designs Salon & Spa:

To process any service or product purchase charges that I incur, or that is incurred by a member of my family or authorized guest as indicated below at Divine Designs Salon & Spa and which will include a 3% operational cost surcharge to total services received and/or products purchased until this authorization is revoked in writing;

I understand that this credit card is also provided as a payment guarantee for any reservation made by me or my authorized guest(s) as indicated below and agree to the non-disputable cancellation terms for any such reservation(s) as follows:

For single-service reservations (less than one-hour in length), cancellations are accepted without penalty up to 24 hours prior to any scheduled reservation date and time;

For single-person multi-service reservations totaling more than one-hour in length, cancellations are accepted without penalty up to 48 hours prior to any scheduled reservation date and time;

For group or multi-person reservations, cancellations are accepted without penalty up to 7 days prior to any the scheduled reservation date and time;

I agree that Divine Designs Salon & Spa has my permission to charge my credit card or any gift-card purchased with this credit card, 50% of the estimated cost of any reservation or $25 per hour (or portion thereof) of scheduled services, whichever is the greater, if any reservation is cancelled after the cancellation fee-free period for reservation(s) as noted above and 100% of the estimated cost of any scheduled service(s) for reservations where you or your authorized user or guest fail to check-in for, or complete, any reservation as scheduled without providing prior notice of cancellation within the time period(s) as noted above.

I understand that any gift cards or electronic gift certificates purchased with this credit card are not refundable once purchased and an electronic delivery of any such item fulfills the purchase and delivery criteria of same.

I understand that gift cards must be physically surrendered upon checkout in order to be valid for use and are not replaceable even if lost or stolen.

CARDHOLDER AGREES TO THE TERMS AS INDICATED HERE AND AGREES TO PERFORM THE OBLIGATIONS SET FORTH BY THE CARDMEMBER'S AGREEMENT WITH THE ISSUER.
