You understand you and/or your child are participating in SummerCampAZ’s camp activities some of which may include, trampoline court and park activities, horseback riding, petting zoos, dodgeball, nerf guns, and fiscal activities, that may include anticipated and unanticipated risk that could result in physical and or emotional injury, paralysis, death or damage to self and or to others. Risks may include, but are not limited to slipping and falling, landing improperly, collisions with fixed objects, moving objects, and or other people, which may result in sprains, fractures, brakes, scrapes, bruises, dislocations and injuries to head and neck, and back.
In consideration of the services provided by SummerCampAZ, it’s host owners, directors, and employees and my desire to spectate and/or participate in activities and/or services provided by by SummerCampAz, I will provide my electronic signature to acknowledge my understanding and agreement to the foregoing terms.
On behalf of myself, my spouse, my children, minor child from whom I am and appointed guardian, my parents, I agree to the following statements.
A: I agree to use the camp and its facilities in a safe and responsible manner.
B: I acknowledge that the camp and park may host some events which may employ, and use reduced and or altered theatrical lights, flashing lights, strobe lights, color, smoke and other special effects. I recognize that some individuals have an increased sensitivity to such special effects which can induce seizures, migrants or other ailments leading to serious physical or emotional injury. If I am pregnant, have a heart condition, or am aware of a health concern related to my exposure to such special effects, I will only participate in the event, solely and exclusively, at my own risk, and that by participating, I waive the right to seek damages for any injury that may occur.
C: I Agree to abide by the camp/parks rules, instructions, and the directions of the park employees and representatives, wherever I acknowledge that those rules, instructions, and directions are indeed to promote the safety of both myself and others. My failure to and/or refusal to abide by these rules, instructions, and directions can lead to an immediate revocation of my rights to use the camp/park and its facilities, without any right to a refund. I authorize the camp/park employees and representatives to obtain and secure, on my behalf, emergency medical treatment and transportation, when deemed appropriate by the camp/park employees and representatives, and I agree to assume at my expense all cost of emergency medical care and transportation.
D: I agree to fully and forever wave, release, and discharge camp/park owners, directors, and employees from any and all claims, actions, cause of action, demands, judgment, damages (including compensatory, general, special, consequential, exemplary and/or punitive), liability or obligations of any nature or kind, whether known at the time, or may become known later, which occur on account of, or in any way arise out of or in connection with: my activities within the park, the activities within the park by others, operations of the camp/park by owners, directors, managers, and or employees, and use of any equipment withing the park, regardless of whether such claims are found in whole or in part alleged negligence.
E: I agree to identify and hold Camp/Park owner’s, directors, managers, & employees harmless from and against any and all losses, liabilities, claims, obligations, cost, damages, and or expenses where so ever, including, but not limited to, and in all attorney fees, damages and or judgments directly or indirectly arising out of, or relating to my act or omission while participating in any activity in the camp/park.
F: I agree to accept and assume all the risks associated with attending and participation in this camp/park’s activities and represent that my participation in these activities is purely voluntary.
G: I fully understand that participating in the activities within the camp/park involve physical exertion, and accordingly represent that I am sufficiently in good health to participate in these activities within the camp/park: I do not have any pre-existing physical or medical conditions, including without limitations, pregnancy, orthopedic problems, including back problems, heart problems, and/or breathing problems, that might impact or worsen by my use of the park, and will not use the park and its facilities while under the use of drugs, alcohol or medications that may impair my physical abilities and or judgment.
H I agree to watch the camp/park safety video before participating in any activity, attempt only activities that I feel I may be capable of performing safely, and stay in an area that will not place me in danger.
I: I certify that I have adequate insurance to cover any injury or damage that I may cause or suffer while participating in activities within the camp/park or if not, I agree to bear the cost of such injury or damage to myself and others.
J: I authorize Camp/Park owner and its successors to capture my image, likeness and sounds in photography, videotapes, recordings and/or other forms of media images. I acknowledge that the park owner will own such images and I grant permission, without compensation, for Camp/Park owner, or any affiliated party of the trampoline park, summer camp AZ, and or any of their directors, to copyright, display, publish, dispute, use, modify and print such images in any lawful manner, including without limitations, and publications, advertisements, brochures, websites, social media and other electronic displays and transmissions thereof.
By providing my electronic signature, I acknowledge my understanding and agreement in the foregoing terms:
*All campers are required to purchase a camp shirt and DEFY grip socks. All campers are required to come to camp in Camp shirts and DEFY grip socks; bring lunch or money for lunch and we recommend snacks and a refillable water bottle. Campers can bring extra money for the concession stand when available, but it is not required. We are not responsible for any lost, stolen, or left personal property. Students should not bring electronic games, phones, and/or toys unless they understand that it is their property and therefore their responsibility to take care of it. Check in time is between 8 am and 9 am, and all students should check into camp no later than 9 am as our activities will start shortly thereafter. All parents/guardians are required to sign their kids in and when they leave.
When you enroll your child in our camp, you will be surrounding them with other great and positive kids doing positive activities. They will learn the importance of good sportsmanship, fairness, and getting along with others. Your child will start to act more responsibly, follow rules, and take responsibility for their actions. We have the best teachers and we thank you for enrolling your children in our camps.
By submitting this form, I give permission to SummerCampAz to withdraw the total for this camp from my Credit Card.
