*All one week only camper requjired to purchase a camp uniform and camp Shirt. Campers That are registering for more than one week, are required to have the safety package. All studend are required to come to class in Camp shirts and Sneekers. They are to bring with them in a backpack or carry bag, thier Karate Uniform, Swim Suit, Towel, Sunblock, a Lunch, Snack, and a refillable Water Bottle. Students can bring extra money for some of the fieldtrips, but it is not required. We pay for almost all events that are part of the Jaz Family Camp. Jaz Family Sport and APSK Martial Arts will not be responsible for any Lost, Stolen, or left personal property. Students should not bring electronic games or toys unless they understand that it is their Property and therefore there responsibility to take care of. All students should be to camp before 9:15am. All of our Parents are asked to please come in with thier children in the mornings. and coming in to pick them up. We are a family camp and we would like to get to know you as well. Jaz Family Sports Camp is no day care!When you enroll your child in our camp, you will be surrounding them with other great and positive kids doing positive activities. They will learn the importance of good sportsmanship, fairness, and getting along with others. Your child will start to act more responsible, follow rules and take responsibility for their actions. The student/client understands that they are entering into an agreement with Jaz Family Sport and Fitness / APSK Martial Arts
The student/client represents that he/she is physically fit to participate in the prescribed course of training
The Student/client understands that Jaz Family Sport and Fitness & APSK Martial Arts, Jerold and Rafaela Shaw and all employees and helpers associated with our facility will not be held liable for any injuries, damages or other loss while participating in this camp.
The student agrees to be included in any filming, portrayals, brochures or advertising of APSK Martial Arts Academy without compensation or royalty as long as they are not fraudulent or deceptive.
I hereby authorize Jaz Family Sport and Fitness, APSK Martial Arts to seek immediate medical treatment for my child listed above, if a medical emergency arises while he/she is doing an APSK Martial Arts activity. I also authorize a physician to perform any emergency treatment necessary,
Jaz Family Sport and Fitness, APSK Martial Arts is hosting a martial arts camp and not a daycare. Parents may pick their children up at any time during the day or class time! All students may come and go at their own free will. All Students should bring a sack lunch, and water every day, except Friday.
APSK Martial Arts is providing a camp program that includes martial arts, dance, tumbling, fitness training and free field trips. All participants must wear martial arts camp t-shirts while participating on these days.
The undersigned hereby releases the promoters, the teacher, and the establishments that are campers might attend, free of harm APSK Martial Arts, Jaz Family Sport and Fitness, Jerold Shaw Jr. Rafaella Shaw, City of Gilbert, City of Mesa, Or anyone connected in any way with this Camp including their directors, officers, employees, or agents of any injuries which could be sustained by participation in this summer camp. I fully understand that if any medical treatment is given to me or my child, such treatment will be first aid type only. I waive any compensation in regards to any photo's or video taken of me with which might be used for publicity, promotions or television showing or other media outlet showings. No Refunds or charge back will for any reason. If a camper is under 18 years of age, this form must be signed by a parent or adult legal guardian.
By sumitting this form, I give permission to Jaz Family Sport and Fitness or APSK Martial Arts to withdraw the total for this competion from my Visa or Master Card.
