  • Agenda item requests are to be submitted at least 4 weeks before the meeting for assessment and investigation by the Advisory Committee Meeting Facilitator in addition to the agenda.
  • Items received after the agenda deadline will be assessed for the next advisory committee meeting.
  • A separate form must be completed for each topic/agenda item.
  • A copy of your agenda submission will be emailed to the email address provided below.
